2.1. Convert Existing Standard Questionnaire into Program Questionnaire

To convert Existing Standard Questionnaires into Program Questionnaires follow the steps outlined below:

1. Go to Evaluations > Questionnaires
2. Click Copy next to the questionnaire
3. Make sure the appropriate department is selected (the department you are logged into will be selected by default.)
4. Scroll to the bottom of the page and click Convert to Program Evaluation Form
5. Read the popup and click OK
6. Click Copy

The form is copied into the Program Questionnaire list (Evaluations > Program Evaluations > Questionnaires) and can be edited and used in program evaluations.
